Medication Management

RISE began offering medication management services in May 2023. 

 

We recognize that medication can play a crucial role in managing mental health conditions and achieving overall well-being. At RISE, our medication management services are tailored to support individuals navigating the complexities of mental health treatment.

The RISE medication management team consists of Board Certified Psychiatric Mental Health Nurse Practitioners (PMHNP-BC).

 

Our services are designed to provide personalized care and support throughout the treatment journey. RISE providers dedicate quality time to each client with evaluations lasting about 1 hour and follow up appointments lasting about 30 minutes.

Service Offerings

Our mental health medication management services include:

Medication Intake/Evaluation: Providers conduct thorough evaluations of clients’ mental health history, current symptoms, and medication regimens to develop personalized treatment plans. Evaluations are about 1 hour.

Medication Monitoring, Adjustment, & Optimization: Our providers work with clients to closely monitor health conditions and client response to medication, making adjustments as needed to achieve desired outcomes while minimizing risks. Providers optimize medication dosages and combinations, ensuring maximum effectiveness with minimal side effects. Follow up appointments are about 30 minutes.

Medication Education and Counseling: RISE clinicians offer personalized education and counseling to clients and their families, providing insight into medication benefits, potential side effects, and strategies for enhancing medication adherence.

Collaboration with Mental Health Providers: With RISE offering both medication management and mental health counseling, our providers are able to easily collaborate with one another to ensure coordinated and holistic care.

 

Supportive Care and Resources: Our team provides ongoing support, guidance, referrals, and resources to empower clients in managing their mental health and medication regimens effectively.
